After more than 40 years of practicing nutritional and functional medicine, I have learned not to become overly impressed by the front of a supplement label.

You know the kind of label I am talking about. It may be colorful, sophisticated, and filled with the latest popular ingredients. It may use words such as advanced, maximum strength, professional grade, or clinically formulated. But when you turn the bottle around and look more carefully, a different story sometimes emerges.

The ingredient may be present in the wrong form. The botanical may not be properly standardized. The dosage may be only a fraction of what was used in the published research. There may be so many ingredients competing for capsule space that none of them are present in a truly meaningful amount.

The label may be impressive. The formulation may not be.

I have worked with thousands of complex patients during my career. I have seen well-designed nutritional products become valuable parts of a patient’s treatment program. I have also watched promising products fall short because the formula was not designed with real clinical practice in mind. The Lesson I Learned From Reviewing Thousands of Labels

Early in my career, it was tempting to evaluate a product primarily by its ingredient list. If a label contained several promising nutrients and botanicals, it seemed logical to assume the formula would be effective. Experience taught me otherwise.

An ingredient name tells you remarkably little by itself. Two products may list the same botanical on the label and still be completely different in their probable clinical effects.

  • One may use the root, while another uses the leaf.
  • One may use a carefully characterized extract, while another uses inexpensive powdered plant material.
  • One may be standardized to the constituents used in successful human trials. Another may be standardized to a convenient marker compound that has little relationship to the desired effect.
  • One may contain a clinically relevant dosage. The other may contain just enough to place the ingredient on the label: often referred to as "fairy-dust" dosing.

This is why a sophisticated label is not the same thing as a sophisticated formula.

1. Before Asking Whether It Works, Ask Whether It Is the Right Ingredient

Ingredient identity must come first. For a botanical, this means knowing more than its common name. Ideally, the manufacturer should be able to verify the genus and species, the plant part used, and the method of authentication.

The National Institutes of Health (NIH) Office of Dietary Supplements emphasizes that botanical identity must be properly assessed and documented. Testing should be capable of distinguishing the intended species from related plants and detecting possible substitution or adulteration [1]. Quality does not begin when the capsules are filled. It begins in the field, at harvest, and during ingredient selection. By the time a poor-quality raw material reaches the encapsulation machine, it may already be too late to rescue the product.

2. Standardization Should Mean Something Clinically

The term standardized extract sounds reassuring. Sometimes it is; sometimes it tells only a small part of the story. A label may state that a botanical is standardized to a certain percentage of one marker compound. That can be useful, but it does not necessarily define the full activity of the extract.

A plant may contain dozens or even hundreds of naturally occurring compounds. High-quality botanical research looks beyond one isolated marker and considers the broader chemical profile of the extract. When I review standardization, I ask:

  • Are those compounds believed to contribute to the clinical effect?
  • Is the standardization similar to that used in human trials?
  • Does the extraction process preserve the desired chemical profile?

A percentage on a label may look scientific, but that does not necessarily mean the ingredient resembles the material used in successful research. For example, our Cholesterol Complete formula uses specific standardized extracts backed by evidence for cardiovascular wellness.

3. Do Not Be Seduced by Extract Ratios

You may see botanical ingredients promoted as 10:1, 20:1, or even 50:1 extracts. Those numbers can sound impressive, but a 20:1 extract is not automatically twice as effective as a 10:1 extract.

Plant-to-extract ratios can be misunderstood or used without enough context. The final product is influenced by the quality of the original plant, the solvent used, and the chemical compounds that remain in the finished extract [6]. A large ratio can create the appearance of potency, but it does not necessarily establish clinical value.

4. Clinical Dosage Matters: Perhaps More Than Anything Else!

This is one of my greatest frustrations when reviewing supplement labels. A formula may contain 15 or even 20 attractive ingredients. At first glance, that seems impressive. But then I begin doing the math.

There is only so much material that can fit into a capsule. When capsule space is divided among too many ingredients, especially in a proprietary blend, many of them may be present at only a small fraction of the dosages used in clinical research.

An ingredient cannot reasonably be expected to reproduce the results of a study when it is present at only a small fraction of the studied dosage. When evaluating dosage, I ask:

  • Was the evidence from a human clinical trial?
  • What exact ingredient or extract was used?
  • Is the dosage high enough to be clinically meaningful?

If the dosage is merely "window dressing," the patient is unlikely to see the results you are both hoping for.

5. Bioavailability: The Milligrams on the Label Are Only the Beginning

What a patient swallows is not necessarily what the body absorbs. Bioavailability is influenced by water or fat solubility, stomach acidity, and digestive function.

Researchers have argued that botanical development should incorporate many of the same principles used in pharmaceutical development, including identification of pharmacokinetics and clinical effectiveness [7]. Sometimes a smaller amount of a well-characterized, better-absorbed ingredient: like the stabilized forms found in Glucose-IR: may provide greater physiological exposure than a much larger amount of a poorly absorbed form.

6. Why BioSpec Formulas Are Designed to Be Stackable

Clinicians who treat complex patients understand that most patients do not arrive with one isolated problem. A patient may simultaneously struggle with insulin resistance, chronic inflammation, and poor sleep. Trying to place every possible ingredient into one enormous formula is rarely the best answer.

BioSpec formulas are instead designed to be stackable. A stackable system allows the clinician to select complementary formulas based on the patient’s actual needs. A properly designed stack should:

  • Minimize unnecessary ingredient duplication.
  • Avoid excessive cumulative dosages.
  • Address complementary physiological pathways.
  • Maintain reasonable capsule and financial burdens.

Stackability does not mean placing every patient on every product. It means giving clinicians a flexible set of professional tools that can be combined logically and responsibly.

When I evaluate a new or existing formula, I use the following hierarchy of evidence:

  1. Human clinical evidence: Well-designed human trials receive the greatest weight.
  2. Supporting mechanistic evidence: Cellular and animal research help explain mechanisms but do not replace human data.
  3. Ingredient specificity: The species, plant part, and extract should match what was studied.
  4. Clinically rational dosage: The amount must be capable of providing meaningful physiological support.
  5. Safety and compatibility: The formula must be evaluated for contraindications and interactions.
  6. Real-world usability: The dosing schedule and cost must remain realistic for patient adherence.

A Quick Label Checklist for Clinicians

Before recommending a professional supplement, ask these questions:

  • Is the exact ingredient identified? Look for the chemical form and botanical species.
  • Is there meaningful standardization? Determine if it relates to the desired clinical effect.
  • Does the dosage resemble the research? Do not be swayed by long lists of underdosed ingredients.
  • Is bioavailability addressed? Consider how the body will actually utilize the nutrient.
  • Is the product manufactured under appropriate quality controls? GMP is essential, but good formulation is what drives results.
